A fantastic issue! Thanks! I especially appreciated THE ROLE OF THE ECONOMIST IN ECONOMIC DEVELOPMENT by Christopher J. Coyne and Peter J. Boettke.

On the role of endogenous institutions, I think the issue of individual/collective action is important. The culture in most poor countries is very much collectivist in that family patriarchs and village leaders make important decisions for everyone. Those societies don't value individual achievement, but rather see it as "showing off" and arrogant. I would expect cooperatives would work better in most poor societies than trying to instantiate Western-style entrepreneurship.
